Understand:

    • Formal investigations
    • Statistical content
  • The types of records and statements
  • The types of written reports
  • The need to sign and date all types of records
  • The use of decision logs as a record
  • The need for personnel to provide witness statements
  • The benefits of recording the content of incident debriefs

Practical application

Record all relevant incident information in an appropriate format

Demonstrate the ability to:

  • Record relevant information that is:
    • Accurate, clear and unbiased
    • Captured in an appropriate format
    • Signed and dated where possible
  • Consider all types of records, including:
    • Documents
    • Voice recordings
    • Photographs and videos made by personnel
    • Photographs or videos from on-site equipment or CCTV
    • Sketch plans and diagrams

Collate and secure records from the incident and witness statements

Demonstrate the ability to:

  • Use templates if provided by the service
  • Focus on objective and personal recollections
  • Try to discourage opinions or unfounded conclusions
  • Obtain and record names and locations of witnesses

Record statements from relevant people as soon as practicable

Keep decision logs, that include a record of decisions and the rationale for actions
